Note: If you are facing problem in opening the Google form, please send the detail of the teachers at email julms@jammuuniversity.ac.in in Excel Sheet with the following as heading in the first row:

username       firstname       lastname       email       phone1       password      department      institution

username must contain small letters only. <SPACE> is NOT ALLOWED. Teacher can use employee id e.g. t-472 or an email id or any other username to ensure uniqueness.
